Our client is seeking a dedicated Primary Care Physician to join their growing team in the Chicago area . This full-time, in-person role is ideal for a physician passionate about providing high-quality care to patients with complex, chronic conditions and a strong commitment to serving Medicaid and Medicare populations. Who You Are
A Board-Certified Internal Medicine or Family Medicine Physician licensed in Illinois.
Experienced in Primary Care with at least 2-3 years per position in prior roles.
Passionate about providing comprehensive care to underserved populations.
Skilled in managing patients with multiple chronic conditions and complex medical needs.
Comfortable working with behavioral health and addiction challenges in a collaborative, patient-centered environment.
Open to occasional home visits for patient care (not required but preferred).

Your Role
Provide comprehensive primary care to a panel of approximately 250 patients , seeing 10 patients per day .
Lead and collaborate with a multidisciplinary care team including case managers, behavioral health specialists, pharmacists, and nutritionists.
Develop transitional care plans and coordinate discharges from hospitals, ERs, and skilled nursing facilities .
Accept same-day/next-day urgent appointments to prevent hospitalizations.
Utilize eClinical EMR for patient documentation and care coordination.
Educate patients on chronic disease management and the importance of advanced directives .
How You Qualify
MD or DO degree with Board Certification in Internal Medicine or Family Medicine .
Active Pennsylvania medical license .
BLS certification .
At least 15 years of hospitalist experience (preferred but not required).
Strong problem-solving skills and ability to engage patients resistant to treatment.
Comfort working in a high-touch, value-based care model .
